What is Medication Therapy Management?
Medication Therapy Management: Community Pharmacists' Definition- Medication Therapy Management (MTM) is a healthcare service provided by a qualified healthcare provider, such as a pharmacist, to help patients and/or caregivers optimize the benefits of medication, minimize risks associated with medication use, and maximize the cost-effectiveness of medication therapy. Preferably, MTM services are conducted one-on-one and face-to-face between patient and healthcare provider. MTM services do not need to be provided in conjunction with the provision of a medication product. MTM can include the following services: Performing or obtaining assessments of the patient's health, including known allergies, laboratory measurements, and medical conditions Performing a review of all prescription and over-the-counter medications taken by the patient to identify, resolve, and prevent medication-related problems Helping patients understand the appropriate use of their medications Helping patients properly take their medications Formulating a medication treatment plan Selecting, initiating, modifying, or administering medication treatment Monitoring and evaluating the patient's response to medication Accurately recording the services delivered to the patient and communicating necessary information to the patient's other healthcare providers

If my doctor and pharmacist have already reviewed my medications, why do the reviews take so much time? Reviewing my medications should only take a few minutes?
The unique medication needs of seniors require time to review. Our services include a review of all medications with recommendations of how and when they should be taken. After the senior reports how they feel, an evaluation of possible side effects will be completed and reviewed with the senior and their caregiver and also shared with their physicians. A senior’s health status may affect the response they get from their medications. A review of their health and the conditions for which they are being treated, how they feel and their complaints are reviewed to determine if their medications may be causing, aggravating, or preventing resolution of these problems. We provide information to the physician and work with the physician to maximize the senior’s response to the medications they take.

Will my physician be receptive to your reports and recommendations? Will my physician need to know that I use your services?
Many of today’s physicians have had the opportunity to work with clinical pharmacists who provide feedback to them about the medication that their patients are taking. We begin our reviews by providing an overview of our services to the senior's primary physican and others as requested. Physicians may request or decline receipt of the Medication Therpay Managment reports. We will provide information as requested by the physician. Our goal is to assist seniors in the proper use of medications and to prolong their independence by offering opportunities to optimize response to medications. Physicians welcome information to improve the healthcare outcomes of their patients.
