Whether it be your parent, spouse, sibling or client, you don't want medication-related problems to result in hospitalization, nursing home placement or worse. Please contact us today to have the medication regimen and related health record reviewed by a Board-Certified Geriatric Pharmacist. We will send you a packet of forms to complete. One of our pharmacists will review the medication regimen, health history, medical and laboratory information. A customized report will be prepared with recommendations that can be presented to the patient's physician(s) to optimize medication therapy. For a more robust review, consider an in-home visit. Additional screenings can be performed during this consultation. Compliance issues are effectively addressed during in-home consultations.  We work with boomers and their parents, physicians, professional Geriatric Care Managers, Elder Law Attorneys and most other senior care providers.

Do you currently take 5 or more prescription medications?

Do you take 12 or more medication doses each day?

Do you take any of the following medications?

  • carbamazepine (Tegretol)
  • lithium (Eskalith)
  • phenytoin (Dilantin)
  • quinidine (Quinidex)
  • warfarin (Cumadin)
  • digoxin (Lanoxin, Lanoxicaps)
  • phenobarbital
  • procainamide (Procanabid, Pronestyl)
  • theophylline (Theo-Dur, Theo-24, Slo-Bid, Theospan, Uniphyl)
Are you currently taking medication for 3 or more medical conditions?

Have your medications or the instructions on how to take them changed 4 or more times in the past year?

Does more than one physician prescribe medication for you?

Do you get your prescriptions filled at more than one pharmacy?
